L'idronefrosi solitamente colpisce un solo rene (idronefrosi unilaterale) anche se può capitare che vada a interessare entrambi i reni (idronefrosi bilaterale). Il rene colpito da questa condizione, qualora non trattato opportunamente, rischia di andare incontro ad atrofia (riduzione delle funzioni e del volume).

Le idronefrosi di primo grado (spesso definite pielectasie) sono di riscontro molto frequente nelle ecografie eseguite in gravidanza e costituiscono, nella maggior parte dei casi, semplici varianti morfologiche della pelvi renale, l'organo dove transita l'urina prodotta dal rene.
